Care Management

 Agape Senior Solutions’ passion is to advocate and educate those who want to be a part of assisting you or your loved one in their individualized journey. 

Think of a Care Manager as a paid family member. As a prior Executive Director of  senior care communities we provide  a unique “behind the scene” perspective, while honoring your ideal health care practices. When it comes to making senior care decisions for a loved one, families may be intimidated by researching and trying to understand which services their loved one needs, that’s where care managers can help!  We can present  a variety of options, helping families choose the plan that works best for their situation.

Because of our  experience, we offer knowledgeable, yet realistic expectations on collaborating with the facility, physicians, and other health care providers in the interest of the client. 

​As Care Managers we have no emotional connections  or complicated family history to overcome. Our objectivity is valuable when it comes to navigating tough family conversations about a loved one, thus we can also assist with mediating.

Guardianship

Guardianship is a legal process in which a person’s rights are delegated to someone else to act on their behalf. This is a protective measure for individuals who cannot make decisions, cannot communicate decisions, or cannot act in their own best interest.

These vulnerable people may be:

  • A person with an age-related decision impairment;
  • A person with a traumatic brain injury;
  • A person with a significant developmental or intellectual disability; or,
  • A person with a substance abuse or mental health issues.

Guardianship and incapacity must be determined through a judicial procedure when a person lacks capacity to manage some or all of their property or to meet essential health, safety, or welfare requirements. If determined to be incapacitated, the court may delegate the persons rights to a guardian to act in the persons best interest while maximizing the person’s independence. Guardianship is a last resort. All other alternatives to guardianship should be explored.

The intent of guardianship is to better protect the incapacitated person while preserving the person’s rights.
